IHSAA PHYSICALS MUST BE UPLOADED TO FINAL FORMS ACCOUNTS SET UP BY PARENT/GUARDIAN – no other forms (PE forms, My Chart receipts, immunizations, etc.) will be accepted.  NO hard copies will be collected.  It must be an upload to the account.  This must all be completed prior to 1^st day of tryouts.
